Home Care Services

Home Health is a team effort involving the patient, doctor, family and the home health staff.

Home Health makes it possible for the recovering or chronically ill patient to receive needed healthcare services in their home. This often enables the hospitalized patient to return to their home environment sooner and with less of the stress associated with the transition from hospital to home.

Perry County Memorial Hospital Home Health is the only hospital-based home health agency in our community, and offers a variety of programs to ensure that our patients' needs are being met.

Social Workers assist patients and families to adjust to, or deal with problems that can greatly affect their care and ability to regain their previous health status.

Registered Nurses develop a physician-authorized care plan that is reviewed with the patient and the patient's family. The registered nurse provides any skilled nursing care that might be needed and supervises the home health aides who are assigned to provide the patient's personal care.

Home Health Aides are trained to work under the supervision of a registered nurse. They help the patient with their daily routine of personal care, perform limited household tasks and assist with prescribed exercises. Home Health Aides are trained and certified through the State of Indiana.

Speech Therapists focus on helping the patient regain their maximum level of speech after a stroke or other speech debilitating condition. This therapy is also used in the treatment of swallowing disorders.

Physical Therapists can help patients regain a maximum level of health and independence, while recovering from a debilitating injury or illness. After doing a thorough evaluation, the therapist will develop the appropriate rehabilitative exercise program.

TO QUALIFY:

	You must live in Perry or Spencer County.
	You must be homebound, but you do not have to be bedfast.
	You and/or your caregivers must be willing and capable of learning self-care skills which will allow a collaborative effort with the home health team.
	You must have an order from your doctor.
