Rehabilitation Services

Perry County Memorial Hospital’s Rehabilitation Services offers a wide range of comprehensive services within physical, occupational, and speech therapy. Our rehabilitation team consists of some of the region’s most qualified speech-language pathologists, physical therapists, and occupational therapists to serve you. Evaluation and treatments are available to both pediatric and adults. To ensure we meet your needs, we are available in outpatient, inpatient, and in home health settings.

Referrals from the physician or nurse practitioner of your choice are required for PCMH occupational and speech therapy. Physical therapy services may be available without an order from your doctor with certain commercial insurances (an order is always required for Medicare and Medicaid payers). Appointments are required for PCMH rehabilitation services, but same day appointments may be available for your convenience.

Hours for PCMH Rehabilitation Services are 7a.m. – 5:30p.m. Monday-Friday on the main PCMH campus, with additional hours available upon request. Physical therapy appointments may also be available for you convenience at the Perry Count Family Practice location in Leopold.

Case Management

Case management is a collaborative process in which a case manager works with clients to ensure they obtain the proper health care in the most cost-effective manner.

Services Include: Utilization Review, Social Services, Swing Bed, Discharge Planning and we are contracted with InterQual.

Physical Therapy at Perry County Memorial Hospital

At Perry County Memorial Hospital, we offer a fully equipped physical therapy department to treat a variety of conditions.

Our physical therapists are movement experts who optimize quality of life through prescribed exercise, hands-on care, and patient education. They teach patients how to prevent or manage their condition so that they will achieve long-term health benefits. Our PTs examine each individual and develop a unique plan, using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability. In addition, they work with individuals to prevent the loss of mobility before it occurs by developing fitness- and wellness-oriented programs for healthier and more active lifestyles.

Occupational Therapy at Perry County Memorial Hospital

Improving the independence of your everyday activities.

PCMH occupational therapists help people across the lifespan to do the things they want and need to do through the therapeutic use of daily activities. Our OT’s enable people of all ages to live life to its fullest by helping them promote health, and prevent—or live better with—injury, illness, or disability. Common occupational therapy interventions include helping children with disabilities to participate fully in school and social situations, helping people recovering from injury to regain skills, and providing supports for older adults experiencing physical and cognitive changes.

PCMH Speech Therapy Services

Enhancing your ability to communicate and be heard.

PCMH speech-language pathologists (SLPs) work to prevent, assess, diagnose, and treat speech, language, social communication, cognitive-communication, and swallowing disorders in children and adults. They assists patients with swallowing or speech impairments that can be associated with hearing loss, strokes, brain injuries, birth defects or a variety of other medical diagnosis. Our SLPs are also highly trained to teach strategies to preserve communication and cognitive function for as long as possible for individuals with dementia.

Our speech-language pathologists can evaluate and treat the following in both pediatrics and adults:
